For those facing a life-limiting illness and their loved ones, our compassionate team of experts provides guidance and support, focusing on quality of life with dignity, peace and comfort.
Backed by decades of dedication, each of our hospice care providers includes a medical director, nurses, social workers, certified hospice aides, chaplains, trained volunteers and bereavement counselors. Our care team features a palliative care clinician certified in hospice and palliative care by the Hospice and Palliative Nurses Association. These seasoned professionals and volunteers deliver individualized care for each patient and family. Hospice is available 24 hours a day, 7 days a week, 365 days a year, and is provided wherever the individual needing hospice is living.
Palliative Care Services
We offer a range of palliative care services, from single-session palliative care consultations to a specialized pediatric palliative care program, to optimize quality of life through expertise in pain and symptom management and guidance in medical decision-making for people living with serious illness. Services may include:
- Emotional and spiritual support
- Setup of in-home care settings
- Pain and symptom management
- Assistance with light housekeeping and errands
- Professional therapies and dietary support
- Companionship
Pediatric Palliative Care
Pediatric palliative care goes beyond pain and symptom management, including extensive services similar to traditional hospice care for children and young adults aged 22 and younger, who have been diagnosed with a life-limiting, serious illness, and their family members. Our no-cost services for eligible children and their families help reduce physical discomfort, improve quality of life and provide additional support.
Compassionate Hospice Care
Our residents are like family. When it is time for care to focus on comfort, our compassionate hospice partners provide hospice care right here where our residents are living. Hospice services include coordination of specialized equipment and medications, palliative care, complementary therapies, and the support families need throughout this difficult time.
Bereavement Services
Everyone experiences loss in their own way, yet no one should have to grieve alone. Our comprehensive hospice care includes free bereavement services for those mourning the loss of a loved one. Support is provided through individual or group sessions to encourage connection with others facing similar challenges, offer guidance for those navigating the grieving process and provide memorial services to honor those who have passed.
Our bereavement team also offers community education programs and in-service training programs for professionals on the topics of grief and loss. Services include:
- One-on-one support
- Support groups
- Programs for adults, teens and children
- Remembrance services
- Journaling/writing bereavement groups
- Sudden loss intervention
